Who uses the Adult Application?
Any adult person who decided to become a member of the Boy Scouts of America must use this Application.
What is the purpose of the Application?
The form is an application for registration with the Boy Scouts of America. The Application helps the chartered organization to select qualified leaders to serve as volunteers.
What documents must accompany the Application?
The applicant should fill and submit the Disclosure/Authorization Form whereby he authorizes the Boy Scouts of America and LexisNexis to procure a consumer report that includes information relating to his criminal history as received from reporting agencies.
How long this form should be kept?
The completed form should be retained on file for three years.
What information should be provided?
The applicant should provide the following information: full name, mailing address, home, business, cell phones, date of birth, ethnic background, driver’s license No., gender, social security number, occupation, employer, business address, scouting position (description), e-mail address; information about his scouting background, experience working with youth in other organizations, previous residences for the last five years, current memberships (community, business, labor, professional or religious organizations). The applicant should provide names and phone numbers of his references and answer the following questions: (a) Do you use illegal drugs? (b) Have you ever been convicted of a criminal offense? © Have you ever been charged with, or investigated or arrested for, child neglect or abuse? (d). Has your driver’s license ever been suspended or revoked? (e). Is there any fact or circumstance involving you or your background that would call into question you're being entrusted with the supervision, guidance, and care of young people?
Where do I send the Application?
A unit scouter completes and signs the top copy. He should keep the back copy (applicant copy) and give the remaining copies to the committee chairman with the proper fees. The committee chairman keeps the unit copy, gives one copy to the chartered organization, and forwards the remaining copy to the local council service center for approval and processing.
Council and district scouters complete and sign the application send the proper fee and all three copies of the application to the local council service center for approval and processing.
